Specimen Submission

UPHL offers two primary ways to order tests: 1) via a portal accessed online 2) manually via a paper TRF. Please contact the lab at 801-965-2400 for additional information.

Results are reported via secure e-mail (preferred) or fax.
If you are a new customer, you will need to obtain a provider code that needs to be entered in the requisition form.
